Moderna Vaccine Clinic April 16 at Evergreen

Evergreen Student Wellness, in partnership with the Chehalis Tribe, will host their first vaccination event on campus Friday, April 16 from 8 am until 4 pm.

The Chehalis Tribe has generously offered both the vaccine and their time to support the event.

When: Friday, April 16, 2021, 8 am - 4 pm
Where: Costantino Recreation Center (CRC) Amphitheatre (1st Floor Entrance) 
Appointment: You must sign up for an appointment to receive the vaccine.

People over the age of 18 are eligible to sign up to receive the vaccine. There are limited spots available, and you must sign up to receive a vaccine at this event. 

If registration is full, you will be added to a waitlist. You will be contacted in the event of cancellations or no-shows. 

Everyone who receives their first dose at this clinic are automatically scheduled to receive their second dose in one month on May 14 at Evergreen. 

Both first and second doses of the Moderna Vaccine are being administered. If you are receiving your second dose, please bring your vaccination card to verify you are eligible on this date, and that your first dose was Moderna. 

As a reminder, the Moderna vaccine is a two-dose vaccination, spaced 28 days apart, approved for individuals ages 18 and older. Learn more about the Moderna vaccine.
