Frequently Asked Questions

General Information: This form consolidates several previous processes and customer instructions on one form.  In general, the new employee's supervisor will complete and submit this form to the Support Center well in advance of the new employee's first day of work.  Normally, this should be submitted at least 3 working days in advance, but if new equipment or wiring is required, this request should be submitted at the earliest possible opportunity as there may be considerable delays in obtaining these resources.

The employee should review page one and sign at the bottom of this page when they accept an offer of employment from the college.  Our intent is to have all required computing and telecommunications resources ready for the new employee by the time they begin their new job.

Section 1: Applicant Information
This section is fairly straightforward.  Please fill in all blanks, being certain to include the applicants starting date.

Section 2: Applicant Acknowledgment
The new employee should read and understand this section prior to signing and dating this page.  This signature is required in order to provide new employees with access to college electronic information resources.

Section 3: General Resources
This is where the supervisor should detail any and all special resources requirements.  We assume a basic level of services for each specific to each work unit (network logon, email account, department shared network files) but any additional resources should be requested here.  In the event that an additional telephone is required for this new employee, you are requested to contact the college Telecommunications Manager at extension 6315.

Section 4: Desktop Computer Hardware and Software
This section provides an opportunity to communicate which existing or new computer workstation the new employee will be using.  In addition, it provides a section for telling us if this computer currently has or needs the required software to access the Banner System (student records) or other administrative information systems.

Section 5: Banner Systems
This section is for specifying the new employee's needs in accessing the student records system and provides a place for the supervisor to detail which type of access level is required for the new employee.  Note: The supervisor must indicate which type of access level they are assigning the employee.  Each access level requires a supervisor or dean's signature as well as the signature of the required data steward indicated on the form.  In addition to this form, each employee will be asked to read and complete a FERPA form when receiving initial training on using the Banner system (contact Administrative Computing for more information).

Section 6: Finance System
Access to the college's financial records system requires a supervisor or dean's signature as well as Janet Thompson's signature.

Section 7: Other Administrative Systems
This section provides an opportunity for supervisors to request access to additional administrative information systems as detailed on the form.  Once again, each system requires a supervisor or dean's signature as well as the signature of the required data steward indicated on the form.

Submitting the Completed Form:
The completed form should be submitted to the Technical Support Services, located in Library Room 1806 between the hours of 8:00 to 12:00, and 1:00 to 5:00.

This form should be used for all new faculty or staff members.

What should I do if I want to request a change in accounts or resources for an existing employee?
This form is not required for changing, adding or deleting accounts or resources for currently employed faculty or staff members.  To make these changes, contact Technical Support Services at extension 6627 or email us at support@evergreen.edu.

Who should fill in this form?
The new employee's supervisor should obtain all necessary information , including the employee's signature, and submit the completed form to Technical Support Services (Library 1806) well in advance of the employees first day of work in order to allow for adequate processing time.
