Students Accepted to CIMAS

Congratulations on your acceptance!

These forms must be completed by all accepted students.

• Address Form
• Housing Questionnaire
• Flight Info Form
• Student Health Insurance Information

Accepted students must also complete and return Conditions of Participation form. If you have already done this, it is not necessary to print out this form again. If you have not, please print, sign and send to the address on the form.

Hard copies of Visa Application Forms from the Ecuadorian Consulate, along with supporting documents, must be completed and will be mailed.
