SPED Practicum Info and Required Forms for current students

Special Education Practicum Request Form
In order to receive your practicum placement, you will need to submit a request form. Please submit the form by either emailing it as an attachment to Loren Petty at pettyl@evergreen.edu or dropping it off at his office in Sem 2 E3131. In addition, with your first request, you must submit the Oath of Confidentiality form and a receipt for your background check and fingerprinting. For subsequent requests, you only need to submit a new practicum placement request form. All requests must be received by the deadline listed below.
We process requests for specific school districts in our region - if you would like a placement in a district other than North Thurston, Olympia, Shelton, Tumwater or Yelm, you will need to provide us with a contact person for the district you are seeking placement in. If you would like a placement outside of the districts listed above, you will need to call Loren Petty at 360-867-6573.
Course/Deadlines:
Three courses in the SPED sequence require a practicum placement. Each practicum placement requires 36 hours of classroom observation and participation. Each practicum has it's own set of requirements - please follow the links below for those requirements. Your practicum placement request form must be received by Loren Petty, Field Placement Officer, by the following deadlines.
Special Education Assessment is offered Fall quarter. Your request form must be received by
September 1st.
Fall quarter practicum requirements
Behavior and Classroom Management in Special Education is offered Winter quarter. Your request form must be received by
November 1st.
Winter quarter practicum requirements
Curriculum and Instructional Strategies for Exceptional Learners is offered Spring quarter. Your request form must be received by
February 1st.
