Media Request Form
Individual contract students and academic programs requiring significant or special use of campus media resources must fill out and submit a Media Request Form. Media Request Forms allow students and faculty to define their equipment and facility usage needs to complete credit-generating academic work. Doing this also allows Media Services staff to allocate access, anticipate usage and manage resources to better serve the Evergreen community. Submit a Media Request Form to the Head of Instructional Media, Lib 1507, or the Media Services Manager, Lib 2504 before the second Friday of the quarter.
