The Office of Institutional Research and Assessment uses several different types of informed consent forms in order to conduct research ethically and comply with The Evergreen State College's human subject policies. If you are conducting reseach involving humans at The Evergreen State College then you will likely need to go through Evergreen's Human Subjects Review process. See the website pertaining to human subject review at Evergreen. You can e-mail Jenni Minner if you are interested in obtaining copies of sample informed consent forms. Any sample consent forms that are used should be adapted to the specifics of your own research. Below is a sample consent for a telephone survey Hello, My name is ______. I am calling from ________ to request your participation in a survey of _____________. Your name was selected from a list of ___________. You may have received an introductory letter and survey several days ago if we had your current address on file. I would like to ask your permission to interview you by phone to find out _______________. Your participation is completely voluntary and confidential. Responses may be published in a public report. No individual's name will be identified with their responses in any reports. Are you willing to answer a few questions about _______? |
