Financial Aid forms may be available in a variety of downloadable formats. If you want to view a form as a PDF document and you do not have Acrobat® Reader® installed on your system yet, please download the free version now. If you fill out a form on your computer, please remember to print it and sign it before submitting it by mail or faxing to 360-867-6576. The Data Sheet was developed to answer your questions about the process of applying for and receiving financial aid at Evergreen. Reading it will help you avoid the most common obstacles to receiving your funds in a timely manner.This form must be signed and submitted before aid can be disbursed. Read it over and be sure you understand it before you return it to us. If you have questions, be sure to give us a call, email us or come on in. The Data Sheet explains that in order to be eligible for aid, you must be a United States citizen or an eligible non-citizen. You also must be fully admitted to The Evergreen State College as a matriculated (or degree-seeking) student (not a “special student" or a “provisionally admitted” student) and eligible to enroll fulltime. That doesn't mean you must enroll fulltime, you must be enrolled and attend at least halftime. Please tell us how many credits you are taking. If you are awarded as a full-time student and you enroll for less than full time, your financial aid (including loan proceeds) may be reduced or cancelled. What is your enrollment status?
You are responsible for notifying the Financial Aid Office immediately if you decide not to enroll, withdraw from Evergreen, take a leave of absence, reduce your credit hours, stop attending, or are granted a change in residency status. On the 10th day of the quarter, student enrollment status is locked (permanently recorded) and used as a final basis for determining financial aid eligibility. If you reduce your enrollment status below that for which aid was disbursed before the 10th day lock, you might owe a repayment. |
