At times, you will need to communicate to us how many credits you earned in previous quarters in order to make sure you continue to receive the funding you need. Use the Credit Verification Form for this purpose.

Please have your professor complete and sign this form to verify how many credits you earned if you are:

• changing grade level and want to apply for more loans, or
• receiving funds for which you need to show that you are making satisfactory academic progress, or
• on satisfactory academic progress probation from the Financial Aid Office and need to verify the credits you received.

If you need to communicate to us how many credits you have earned for another reason, please use this form, too.

Please wait until the tenth week of the quarter before reporting the number of credits earned for that quarter.
