Financial Aid forms may be available in a variety of downloadable formats. If you want to view a form as a PDF document and you do not have Acrobat® Reader® installed on your system yet, please download the free version now. If you can fill out a form on your computer, please remember to print it and sign it before submitting it by mail or faxing to 360-867-6576.

At times, you will need to communicate to us how many credits you earned in previous quarters in order to make sure you continue to receive the funding you need. Use the Credit Verification Form for this purpose.

Please have your professor complete and sign this form to verify how many credits you earned if you are:

changing grade level and want to apply for more Stafford loan, or
receiving the Alaska Loan and need to show that you are making satisfactory academic progress, or
on satisfactory academic progress probation from the Financial Aid Office.

If you need to communicate to us how many credits you have earned for another reason, please use this form, too.

Please wait until the tenth week of the quarter before reporting the number of credits earned for that quarter.
