Financial Aid forms may be available in a variety of downloadable formats. If you fill out a form on your computer, please remember to print it and sign it before submitting it by mail or faxing to 360-867-6576.

Are you changing your level of enrollment from full to halftime? Are you graduating or withdrawing from Evergreen? To communicate these or other changes, please use this Change of Information Form.

Part I allows you space to convey your request or information.
Part II is an easy way to tell us how many credits you will enroll in (if you will be on leave for a term, mark "0" credits per quarter). Unless you tell us otherwise, we will assume that you are enrolling fulltime.
Part III concerns total withdrawal during a quarter and allows two options: you may request that all your financial aid be returned to the funding sources; or you may provide a Last Day of Attendance Memo from each of your instructors so we can compute just how much of your award you actually earned while in attendance. This is a multipurpose form. Ignore any part that does not pertain to your particular situation.
