Financial Aid forms may be available in a variety of downloadable formats. If you can fill out a form on your computer, please remember to print it and sign it before submitting it by mail or faxing to 360-867-6576.
If you have special expenses that we should know about in order to expand your budget for the cost of attendance (COA) at Evergreen, please let us know by completing and submitting the Additional Expenses Form.
Examples of expenses that we might approve include:
- additional room and board costs for your dependents;
- childcare expenses;
- commuting expenses if you travel more than 30 miles roundtrip;
- tuition for extra credits;
- medical or dental expenses;
- class fees;
- special academic materials or equipment;
- and expenses related to studying abroad...
...and you may apply for funds to cover the cost of a computer once during your undergraduate academic career.
Please submit a letter of explanation with your specific Additional Expenses Form. Include information about what you plan to do and when you plan to do it, and please itemize your expenses. We also need documentation of costs in the form of estimates or receipts. We may need a copy of your class description or individual contract. Note the box on the front of the form and be sure you follow each instruction.
