FAFSA Verification is a process used to verify certain information on the FAFSA to ensure its accuracy. Some students are selected for verification by the U.S. Department of Education. Others are selected by the school. Only a certain percentage of students are selected for verification.
TAX DOCUMENT INFORMATION FOR 2013-2014
If we require you to provide a copy of your and/or your parent's 2012 taxes and you cannot or are unable to use the FAFSA IRS data retrieval tool, you must supply us with an official IRS 2012 Tax Return Transcript (It does not need to be signed). Per changes in federal regulations, we can no longer accept copies of your handwritten, printed, or copied 1040, 1040A or 1040EZ forms.

Your FAFSA file can be verified for several possibilities:
- Your file was among the approximately 30% of applications that were selected randomly by The Department of Education
- Your application has incomplete data
- The data on your application is contradictory
- Your application has estimated information on it

Please do not submit these forms unless requested to do so from your SAR (Student Aid Report) or The Evergreen State College Financial Aid Office. If requested, please submit them to your Financial Aid Office to determine your eligibility.
Additional Academic Expenses Request Form (PDF)
Tuition and fees and an allowance for books are included in the basic budget for costs of attendance (COA) at Evergreen. Use this application to add other expenses directly related to your academic program or individual learning contract to your COA.
Additional Family Expenses Request Form (PDF)
In the basic budget for costs of attendance (COA) at Evergreen, living costs are based on expenses of a single individual. Independent students with dependents may be eligible to include additional living costs for dependents and/or childcare expenses. Use this application to add necessary and/or appropriate family expenses to your current budget for attending Evergreen.
Medical Expenses Request Form (PDF)
If you incur medical expenses for conditions that prevent you from attending class or education-related activities during the academic year, you may request that your cost of attendance (COA) at Evergreen be increased accordingly. The expenses must be clearly medical or dental in nature, and the must clearly documented. Determination of medical necessity must be made by a qualified medical professional. Include a statement from your provider that supports your request.
Additional Transportation Expenses Request Form (PDF)
An allowance for transportation is included in the basic budget for costs of attendance (COA) at Evergreen. Use this application to add necessary and/or appropriate expenses to your current commute costs for attending classes or class-related activities.

Change of Information and Last Day Attendance Memo
Change of Information Form (PDF)
Use this form for changes/additional information and requests, change your level of enrollment from full to halftime, and graduate or withdraw from Evergreen. This is a multipurpose form. Ignore any part that does not pertain to your particular situation.
Last Day of Attendance Memo (PDF)
If a student fully withdraws from The Evergreen State College and is a financial aid recipient, then the school and/or the student may be required to return some of the federal funds awarded to the student. If you received aid from Evergreen and/or the State of Washington, then a portion of your refund may need to be returned to the funding source from which it was received. In order to determine what portion of your financial aid you were eligible for, we must have documentation of the last day you attended class or an academic related activity. Use this form to relay that information.

Proof of Independent Status (PDF)
You have indicated on your FAFSA that specific circumstances applies to you that cause you to be considered an independent student. Please check the appropriate category and attach the required documentation listed on the Proof of Independent Status form.
Proof of Dependents Form (PDF)
If you are unmarried and under the age of 24 and you claim dependents on the FAFSA, use the Proof of Dependents Form to give us more information about those dependents. That way, we can make sure you receive funding as an independent student.
Request for Independent Status Petition (PDF)
If you do not meet the federal definition of being independent as described in the FAFSA, you may request that the Financial Aid Office override your status on the basis of documented extenuating circumstances. Complete and submit the Request for Independence Form along with a letter explaining your circumstances. Please include letters of support and other documentation as outlined on the form itself. Your petition will be reviewed by the Professional Judgment Committee in your Financial Aid Office.

Filing the FAFSA
FAFSA is a need analysis form used by the Department of Education to collect each family's personal information in order to calculate the family's ability to contribute to the student's educational costs.The Free Application for Federal Student Aid (FAFSA) is used to apply for Federal and State Financial Aid. The FAFSA is a standardized application including detailed financial and household data which is required to determine eligibility for all financial aid programs. A new FAFSA is released each year and can be completed beginning January 1, prior to the Fall semester you will attend college.

Direct Loan Request Form (PDF)
If you would like to accept a reduced Student Loan amount, you may do so by submitting this form.
Direct PLUS Loan Request Form for Parents (PDF)
This form is to adjust your loan amount only.
Parents must go to studentloans.gov to apply for this loan.
The Parent Loan for Undergraduate Students (PLUS) is a Federal Educational Loan. The parent is the borrower of this loan. At the same time, it is part of the student's financial aid package and students are required to meet Satisfactory Academic Progress requirements. The amount that can be borrowed is based on the cost of attendance less any other financial aid awarded. Please visit our PLUS Loan page for more information.
Direct PLUS Loan Request Form for Graduate Students (PDF) Posted when available
This form is to adjust your loan amount.
Graduates must go to studentloans.gov to apply for this loan.
Graduate and professional degree students are now eligible to borrow under the Direct PLUS Loan Program up to their cost of attendance minus other estimated financial assistance in the Direct Loan Program. While PLUS Loans for Evergreen students may not be requested electronically, Master Promissory Notes must be completed online.Please visit our PLUS Loan page for more information.
Short-Term Loan Application (PDF)
The Short-Term Loan allows students to borrow up to $400 cash one time per quarter when life circumstances prohibit them from having funds to cover costs associated with attending school. This loan cannot be used for living costs between quarters.
Unsubsidized Loan Only Petition (PDF) Posted when available
This form is only for students that have no parental FAFSA information and are therefore ineligible for any financial aid except Unsubsidized Loans.

Verification of Assets- Independent (PDF)
The calculations used to determine a student's eligibility for student financial assistance generally take into account the assets in the student's household. From time to time, when reviewing a student's file to determine eligibility for financial aid, we may need to verify that the information we have is correct.
Verification of Family Members-Independent (PDF)
The calculations used to determine a student's eligibility for student financial assistance generally take into account the number of people in the student's household. From time to time, when reviewing a student's file to determine eligibility for financial aid, we may need to verify that the information we have is correct.
The definition of a family member for the purposes of financial aid is rather specific, so please be sure to read this form over before completing it and returning it to us.
Independent Student Income and Resources Statement (PDF)
The calculations used to determine a student's eligibility for student financial assistance generally take into account the family's income from the previous tax year. If you report minimal income for that year on the FAFSA we need to know how you supported yourself before we can complete the review of the application for financial assistance. Use the Income and Resources Statement above to give us the information we need to assess your total income so we can determine how best to help you with financial aid funding to pursue your education.

If the information you provided on the FAFSA does not reflect your current circumstances, and you would like us to consider revising your expected family contribution (EFC) for the current year, please let us know by completing and submitting the Income Adjustment Form.
