Student Evaluation of Faculty
What is the Student Evaluation of Faculty?
Evergreen faculty members are asked to solicit evaluations of their work from all of their students. Students write the Student Evaluation of Faculty at the end of a quarter, or when leaving the program. The Faculty Handbook requires all Student Evaluations of Faculty received by a faculty member be included in the faculty member's portfolio.
Write the Student Evaluation of Faculty
Go to my.evergreen.edu. Your @evergreen.edu login is required and allows you to save draft evaluations to a secure location, then return and make changes prior to printing final copies.
This application works best using the latest versions of Mozilla Firefox and Microsoft Internet Explorer.

To print your Evaluation, Adobe Acrobat Reader must be installed on your computer.

Want feedback as you write your evaluation?

Where do my completed forms go?
You have two options:
- Students typically bring their Student Evaluation of Faculty to the faculty-student evaluation conference.
- If you prefer not to deliver the Student Evaluation of Faculty to the faculty member in person, you may deliver it to the faculty member's program secretary.
If, for any reason, you want to provide a copy for your faculty member's dean, address an envelope to "Academic Deans, Library 2002" and use campus mail.
