Contract/Internship Form Downloads
Note: EVALUATION FORMS ARE NOW AVAILABLE ON THE WEB
Need to fill out a self-evaluation, student evaluation of faculty or summative evaluation? Go to Evaluations at Evergreen.
THE EASIEST WAY TO ACCESS, FILL OUT AND PRINT CONTRACT/INTERNSHIP FORMS IS TO PHYSICALLY COME INTO THE COMPUTER CENTER OR LIBRARY AND USE OUR COMPUTERS.
The home form download process is painful at best. If you choose to challenge yourself to the form download and install process please read all the following
instructions carefully.
Evergreen's Contract/Internship forms are MS Word Templates
that require you to already have MS Word installed
(Note:
all versions may not be supported). If you do not have
Word, feel free to stop by the Computer Center
on campus for assistance.

this form so ridiculously difficult to install?
Why don't you just use something easy like a PDF
or web form? We
agree that this form download and install process
is not simple. Hopefully Evergreen will implement a mechanism for these forms as well.
ATTENTION: DO NOT attempt to install these
files if you are working in the Computer Center
or the Library. The forms are already installed.
Need instructions for how to
use forms in the Computer Center?
For installation on your own computer:
Download for PC:
1. Click on your
Word version below and save to the Desktop
to download.*
> For
Word 2000 & 2003
> For
Word 97
2. After downloading, open the zip file by
double-clicking on the icon and open/read the
included instructions file (INSTRUCTIONS.doc or
README.txt) for further installation instructions.
*Note: You may need to right-click directly
on the link above to Save Link As. All the forms
are compressed into .zip format. They all come
as one file you will need to extract into
a folder on your own computer.
If you don't already have WinZip installed,
download it at www.winzip.com.
The evaluation version is free! There is also
a completely free zip program called Seven-Zip.
Once installed, simply double-click on the downloaded
file.

ON ANY OF THE .DOT FILES.

1. Click on your Word/OS version below
and save to the Desktop to download.*

2. After downloading, open the sit file and read
the included ReadMe file for further installation
instructions.

right-click) directly on the link above to download
the file. Mac Forms are compressed in .sit format.
They all come as one file you will need to extract onto
a folder on your own computer. If you don't already
have Stuffit Expander installed, download it at www.stuffit.com.
