How do I change my home mailing address or campus mailstop?

Contact us in person, by phone or by e-mail to request the change. Also be sure that your supervisor and/or timekeeper know your new address so that paperwork that they submit to Human Resources or Student employment has the correct address.

I changed my address on the Gateway - why does my payroll check still have the wrong address?

Payroll is not part of the Gateway system. You need to change your address with the payroll office.

I am an hourly employee (student or non student) when is my payday and what time period does this cover?

You are paid twice monthly on the 15 th and the last day of the month. Hours worked from the 1 st - 15 th are paid on the last day of the month. Hours worked from the 16 th - month end are paid on the 15 th of the following month. If the payday falls on a weekend you are paid the Friday before. If the payday falls on a holiday you are paid the day before (or Friday before in the case of Monday Holidays).

I am Staff or Faculty - If payday falls on a Holiday or the weekend when is payday?

If the payday falls on a Saturday you are paid the Friday before. If the payday falls on a Sunday you are paid on the following Monday. If the pay date falls on a Holiday you are paid the day before (or Friday before in the case of Monday Holidays) . The current years paydates can be viewed from the home page.

How do I change my tax exemptions on my check (form w4)?

You can pick up a new W4 at our office or download the form from this site and drop it off or mail it to us.

What is OASI and why is it being deducted from my paycheck?

O ld A ge S upplemental I nsurance or FICA is your contribution to Social Security. The College matches this amount. Students attending class half time or more that are employed by Evergreen are exempt from paying Social Security taxes.

I am eligible for Earned Income Credit - How do I get the advanced credit?

Download the W-5 form from this site and turn it in to the payroll office.

How do I sign up for or change the amount for my TIAA-CREF supplemental retirement annuity?

If you do not currently have a TIAA-CREF Supplemental Retirement Annuity - you can pick up the enrollment kit at our office or request that we mail one to you. If you currently contribute to a SRA and wish to change the amount - download the Salary Reduction Agreement from this site and state in part C what you wish your deduction to be changed to. Turn this form into Payroll.

How do I change the amount that I contribute to Washington State Deferred Comp plan?

Where do I pick up my paycheck?

The cashier's office is located next to the payroll office on the first floor of the Library in room L-1119. You will need picture ID.

Can I have my paycheck mailed to me?

How do I change the amount of my optional life insurance coverage?

If you wish to increase the amount of your coverage then:

Download the enrollment/change form that you turn into the payroll office and the Evidence of Insurability form that you can mail in yourself or turn it into the payroll office with your enrollment change form and we will mail it for you.

If you wish to decrease the amount of your coverage then:

Download the enrollment/change form and turn it into payroll with your desired changes.

How do I enroll or change the waiting period for my optional long term disability insurance?

If you wish to enroll or decrease your waiting period:

Download the enrollment/change form that you turn into the payroll office and the Evidence of Insurability form that you can mail in yourself or turn it into the payroll office with your enrollment change form and we will mail it for you.

If you wish to increase your waiting period:

Download the enrollment/change form and turn it into payroll with your desired changes.

What month will I get my increment raise?

What do I do if I've lost or haven't received a payroll check?

Contact the payroll office and we will put you in touch with the correct person in the accounting office.

How do I change my beneficiaries for life insurance and retirement?

Download the beneficiaries' forms from this site for life insurance and Department of Retirement Systems retirement plans. To change beneficiaries for TIAA-CREF you will need to contact them directly.

Do I need to do any paperwork if I get divorced?

If your ex spouse is covered under your medical/dental plan you will need to submit to our office a Medical/Dental Enrollment Change Form . You may also wish to change your W4 form and beneficiary forms for life insurance and retirement. (you can download these forms from this site)

Can I add or remove a dependent to my medical coverage outside of open enrollment?

You can add a dependent outside of open enrollment within 60 days of the following events.

Loss of spouse or same sex domestic partners other coverage - Coverage will begin on the first day of the month following the date the prior coverage terminated. Proof of loss of other coverage must be provided.

Marriage - Coverage will begin on the first day of the month after the date of marriage. If the marriage date is the first day of the month, coverage will begin on the marriage date.

Establishment of a qualified same-sex domestic partnership - Coverage will begin on the first day of the seventh month in which the relationship was established and qualifications were met. (for information on the qualifications : http://www.pebb.hca.wa.gov/doc/50-704.pdf

Birth or placement of a child for adoption - Coverage will begin on the first day of the month in which the birth or placement occurred. Coverage for the newborn child will begin at birth. Coverage for a child placed for adoption will begin on the date the employee assumes legal obligation for total or partial support in anticipation of adoption of the child.

Extended Dependents -

Placed in the legal custody of the subscriber by court order;

Living with the subscriber in a parent-child relationship. A parent-child relationship as defined by HCA means the biological parent or step-parent is not residing in the home and the subscriber assumes the parent role;

Not a foster child for whom support payments are made to the subscriber through the Department of Social and Health Services foster care program.
