How do I change my home mailing address?

You can change your address on your my.evergreen.edu account. Once you've logged into my.evergreen.edu, click on the "Employee Services" link and choose the Personal Information tab at the top page. This will bring you to the page that gives you the capability of updating your personal information.

If you choose to contact us in person or send us an email to request the address change, please include your A#. Be sure your supervisor and/or time approver know your new address so the paperwork they submit to Human Resource Services or Student Employment has the correct information.

When is payday and what time period does this cover?

Evergreen employees are paid on the 10th and the 25th of each month. Hours worked from the 1st through the 15th are paid on the 25th and hours worked from the 16th to the end of the month are paid on the 10th of the following month.

When do we get paid if the pay date falls on the weekend or holiday?

If the pay date falls on a Saturday you will be paid on the Friday before.  If the pay date falls on a Sunday you will be paid on the Monday after. If the pay date falls on a holiday you will be paid the day before or on Friday in the case of a Monday holiday. The current years paydates can be viewed on the Payroll & Benefits "paydates" web page.

How do I change my tax exemptions on my check (W4 form)?

Download the W4 form from the Forms and Publications web page. Mail or personally deliver the completed form to the Payroll and Benefits office. The Payroll & Benefits office also has hard copy forms available for you to pick up.

What is OASI and why is it being deducted from my paycheck?

Old Age Supplemental Insurance or FICA is your contribution to Social Security. The College matches this amount. Students attending class half-time or more that are employed by Evergreen are exempt from paying Social Security taxes.

I am eligible for Earned Income Credit - How do I get the advanced credit?

Download the W-5 form from the Forms and Publications web page and turn it in to the Payroll and Benefits office.

How do I sign up for or change the amount for my TIAA-CREF supplemental retirement annuity?

If you do not currently have a TIAA-CREF Supplemental Retirement Annuity - you can pick up the enrollment kit at the Payroll and Benefits office or request that they mail one to you. If you currently contribute to a SRA and wish to change the amount - download the Agreement for Salary Reduction Under Section 403 from the Forms and Publication web page and state what you wish your deduction to be changed to. Turn this form into the Payroll and Benefits office.

How do I change the amount that I contribute to Washington State Deferred Comp plan?

The Deferral Change Request form can be downloaded at the Washington State Deferred Compensation Program/Forms web page.

Where do I pick up my paycheck?

The Cashier's office which is located on the first floor of the Library building. You will need picture ID.

Can I have my paycheck directly deposited into my bank account?

Yes, the form for direct deposit is available at the Payroll and Benefits office or you can download it from the Forms and Publications web page.

How do I change the amount of my optional life insurance coverage?

If you wish to increase the amount of your coverage:

Download the Life Insurance Change form and the Evidence of Insurability form from the Forms and Publication web page.  Mail or hand deliver the completed forms to the Payroll and Benefits office.

If you wish to decrease the amount of your coverage:

Download the Life Insurance Change form from the Forms and Publication web page.  Mail or hand deliver the completed form to the Payroll and Benefits office.

How do I enroll or change the waiting period for my optional long term disability insurance?

If you wish to enroll or decrease your waiting period:

Download the Enrollment/Change form and the Evidence of Insurability form from the Forms and Publication web page.  You can mail or hand deliver the completed forms to the Payroll and Benefits office.  We will mail it for you.

If you wish to increase your waiting period:

Download the Enrollment/Change form from the Forms and Publication web page.  Turn the completed form into the Payroll and Benefits office.

What month will I get my increment raise?

What do I do if I've lost or haven't received a payroll check?

Contact the Payroll and Benefits office and we will put you in touch with the correct person in the Accounting Services office.

How do I change my beneficiaries for life insurance and retirement?

Download the applicable Beneficiary form from the Forms and Publication web page for both life insurance and the Department of Retirement Systems retirement plans. To change beneficiaries for TIAA-CREF you will need to contact them directly.

Do I need to do any paperwork if I get divorced?

If your ex-spouse is covered under your medical/dental plan you will need to submit to our office a Medical/Dental Enrollment Change Form . You may also wish to change your W4 form and beneficiary forms for life insurance and retirement. (you can download these forms from the Forms and Publications web page.

Can I add or remove a dependent to my medical coverage outside of open enrollment?

You can add a dependent outside of open enrollment within 60 days of the following events.

Loss of spouse or same sex domestic partners other coverage - Coverage will begin on the first day of the month following the date the prior coverage terminated. Proof of loss of other coverage must be provided.

Marriage or establishment of a Washington State registered domestic partnership- If the date of the marriage or date of the domestic partner registration is on the first day of the month the coverage will begin on that day otherwise coverage will begin on the first day of the following month.

Birth or placement of a child for adoption - Coverage for the newborn child will begin at birth. Coverage for a child placed for adoption will begin on the date the employee assumes legal obligation for total or partial support in anticipation of adoption of the child.

Who are eligilble family members for PEBB coverage?

Effective January 1, 2011, you may enroll the following family members:
~ your lawful spouse
~ your Washington State-registered domestic partner
~ eligible children up to age 26

Children are defined as:
Your biological children, stepchildren, legally adopted children, children for who you have assumed a legal obligation for total or partial support in anticipation of adoption, children of you Washington State-registered domestic partner or children specified in a court order or divorce decree.

In addition, children include extended dependents in the legal custody or legal guardianship of you, your spouse, or Washington State-registered domestic partner.  You must demonstrate the legal responsibility with a valid court order and the child's official residence must be with the custodian or guardian.
